Follow these easy steps to join our Healthe Habits for Living Network Providers!

STEP 1 - Provider Practice Agreement

Before filling out the registration form and submitting payment, all registrants must read our Provider Practice Agreement in order to join our network of certified providers.

This agreement is intended to: 

• Protect and maintain treatment fidelity

• Protect trademark and copyright laws and HHL’s intellectual property

• Protect patients/clients referred to an HHL network provider

• Protect the investments and specialized training of the certified HHL providers.
