EDUCATION LEVEL: Introductory
SATISFACTORY COMPLETION OF CE COURSES: Criteria for the satisfactory completion of the course will include requirements of signing in/out in the live course, a learning outcomes test and a post-course evaluation.
RECORD KEEPING: Monitored sign in/sign out sheets of course participants, a copy of the certificate of completion, post course evaluation and learning outcomes test will be kept securely for 7 years via confidential filing system (paper and electronically). Lost certificates are available for a fee (replacement – $25) by contacting the HHL Education Division of Healthe, LLC directly for resolution. We will also maintain a record of your course attendance and license number in the event you need to access this information in the future. Verification of name, course location and license number will be required.
CONFIRMATION: A confirmation letter will be sent at least 1 month prior to the course with local hotel and information upon receipt of your registration form and fee. If you do not receive a confirmation 3 weeks before course, please call to verify registration.
VARIABLE CREDIT: is ONLY allowed for the course if it is less than 4 of the 15 live contact hours that is missed. This will be calculated according to the contact hour equation and reflected on an amended certificate which is stored electronically per attendee. If this information is known ahead of time, a certificate with the appropriate credit will be printed. However, if this information is not known ahead of time, a corrected certificate will be either mailed or signed, scanned, and emailed to the attendee. No handwritten changes allowed.
CANCELLATION/REFUND POLICY: Cancellation/Refund Policy: Cancellations must be received in writing at least 2 weeks prior to the course for tuition refund less a $50 administration fee. NO refunds will be made after this date. If you are registered and do not attend an event course credit will not be given and you are still liable for full payment. A substitute attendee can be made at any time before the start of the course.
COURSE CANCELLATION: Healthe, LLC reserves the right to cancel a course up to 3 weeks prior to the course date. In the event of a cancellation, only the tuition fee will be refund in full. Healthe, LLC is not responsible for reimbursement of non-refundable airline tickets, lodging, or other related costs.
ADA: If you require ADA accommodations please contact Healthe, LLC at the time of purchase or at least 2 weeks prior to the course date so that arrangements can be made. Every accommodation is made to the best of our ability.
COMPLAINT RESOLUTION POLICY: Complaint Resolution: Our program is guaranteed to provide high quality effective education. If you attend a 2 day course and do not receive the professional or business benefits described in our literature, or have some other professional complaint, please email the Director at firstname.lastname@example.org within 2 weeks of the course and we will issue a credit voucher that you or anyone in your facility may use toward another HHL Education course. If this is not satisfactory, other possible solutions may be discussed.
Healthe, LLC will not share your personal nor financial information with any outside source, however, you will be added to our marketing list to receive further communication from us regarding upcoming courses and/or products. By supplying your contact information and allowing yourself to be added to our customer files, it is assumed that you agree to this practice unless you specifically indicate otherwise (information sharing permitted by law includes sharing information about our transactions or experiences with our customers).
Please notify us if at any time you wish to update your contact information or remain on our marketing list but discontinue a specific form of communication as permitted by law. If you wish to be removed entirely from our communication files, other than sharing permitted by law, please notify us in writing.
STANDARDS OF CONDUCT
In order to promote uninterrupted production and consistent work practices and to protect life, health, and property to the mutual benefit of the company and anyone representing Healthe, LLCs, standards of conduct must be observed. Certain conduct, which violates company standards, may result in dismissal.
Anyone representing Healthe, LLC is responsible for both integrity and the consequences of his or her own actions. The highest standards of honesty, integrity, and fairness must be followed by anyone representing Healthe, LLC while engaging in any activity concerning the company, particularly in relationships with clients, customers, competitors, suppliers, the public and anyone representing Healthe, LLC.
The company expects that anyone representing Healthe, LLC will undertake any activity while on the company premises or while engaging in the company business that is, or gives the appearance of being improper, illegal, discriminatory against anyone on the basis of gender, age, sexual orientation, disability, socioeconomic or ethnic background, or that could in any way harm or embarrass Healthe, L.L.C., our clients, or our customers.
Anyone representing Healthe, LLC is responsible for complying with this policy. And have signed this document upon hiring.
COPYRIGHTED AND LICENSED MATERIALS POLICY
The materials that are the subject of this agreement shall consist of all proprietary instruction manuals, modules and other written, audio and video materials provided by licensor to course participants. Course participants acknowledge that the rights and title to the licensed materials and any trademarks or copyrights or service marks relating thereto shall at all times remain with licensor. Course participants shall have no rights, no title, and no interest in the licensed materials. All intellectual property rights in and to the licensed material are and shall remain the sole property of licensor.
Specific restrictions on use of licensed materials
Course participant shall be permitted to use and distribute the licensed materials to patients and their healthcare providers subject to the terms and conditions of this agreement, including but not limited to the restrictions set forth. Course participant shall not sell the licensed materials nor transfer to any third party any of the rights under this agreement.
Financial: Jill Hurley is receiving compensation for speaking at the educational activity and receives 100% of services sold by Healthē, L.L.C. Jill is the owner/CEO of Healthē, LLC, and the HHL Education division. Healthē, LLC dba: Healthē Habits for Living, is a private OT/PT practice in the state of Louisiana. Nonfinancial: None

Course Content Disclosure: Jill Hurley, OTR/L has developed a licensed, copyrighted, and trademarked system known as Healthē Habits for Living System. Because there are no other similar products available, this course will only cover information that pertains to the effective and safe use of the above-named system. This presentation will focus exclusively on disseminating the research of, the evaluative and treatment tools, and the administrative tools used by Jill Hurley OTR and her staff at Healthē Habits for Living and will not include information on other similar or related products, nor is there any promotion to purchase any similar or related products.
